My Journey with Efudex in Photos

Front Range Dermatology Associates provided me a list of instructions for applying Efudex and hints for aftercare. In addition, their information sheet briefly mentions a few common symptoms. I am including the link to their instructions as means of comparing their suggestions to my journal of experiences. Below are photos of my Efudex journey.

Before treatment

After treatment: Days 1-7

Day 1 (June 24) No visible reaction. Sensation that something is different. No pain.
Day 2 (June 25) Same as Day 1. Headache. Stomach slightly upset. Fatigued late in the day.
Day 3 (June 26) A little redness around spot on lower left cheek. No pain. No itching. Stomach upset. Fatigued late in the day.
Day 4 (June 27) Discomfort around nose in the crease and over top lip. Chin itchy. Bright red spot beginning to show left side of mouth. Nothing to speak of on nose. Irritation on right side of cheek near nose where I saw a scaly spot.
Day 5 (June 28) More discomfort around creases of nose. Chin itches some. Two prominent bright red spots on sides of mouth and nose. Small red spot showing up on left side of nose. Overall still not inflamed or swollen. Headache and stomach upset.
Day 6 (June 29) Basically same as Day 5. More general redness and beginning to feel tight when cream is cleaned off.
Day 7 (June 30) Stomach upset. Headache late in the evening. General discomfort spreading. Overall redness spreading. Painful to smile or open mouth to chew.

After treatment: Days 8-14

Day 8 (July 1) Lots of stomach upset. Aware of the discomfort all the time. Itches late in the day. Water feels good in shower but immediately feels dry. Mouth feels dry. Slight metallic taste. Headache woke me early this morning. Nagged all day even with Tylenol.
Day 9 (July 2) Stomach upset. Face tightening.
Day 10 (July 3) Stomach upset worsening. Strong metallic taste. Takes an hour for face to completely dry out after shower. Even then it stings to reapply.
Day 11 (July 4) Skin is very tight. Stomach upset was much better. Still have a metallic taste. Throbs horribly when I wash it off in the morning and after shower. I'm waiting about 30 minutes to reapply in the morning and over an hour after shower. Any sooner would be unbearable.
Day 12 (July 5) Face tight all day. Getting much harder to open mouth to eat. No cracking yet. No stomach upset.
Day 13 (July 6) Can't open my mouth much wider than to slide in my toothbrush. Had a headache most of the day. Feels like it might be seeping a little after it’s washed. Skin becomes soft enough in the shower to open my mouth wide as I want but hardens almost immediately after it's dry. Stopped by doc and nurse looked at me. Says reaction is a good one and to try a couple more days at least.
Day 14 (July 7) Pretty much the same as Day 13 with more tightening.

After treatment: Days 15-21

Day 15 (July 8) Very little sleep even with Tylenol PM or Benadryl. Itched and burned a lot through the night. Couldn't find a comfortable position even on my back. I can't tell, but it feels like my face is beginning to sleep at night. I don't see evidence on pillow and I'm not able to wipe away any wetness but face feels cool as if it's wet in places. Extremely tight late in the day and several spots in nose crease and around chin are beginning to flake. Went to bed with worst headache yet.
Day 16 (July 9) Woke several times during the night. Got about 2 hours of sleep. Headache returned in the morning. I think I'm done. Bled in the shower a little. Very seepy after shower and after night application. Cracked and super dry. So tight I can barely open my mouth. Will wash in the morning and start recovery.
Day 17 (July 10) Day 1 post-application: Washed off night application. Waited two hours before using Vanicream. Stung horribly with first application of Vanicream. Headache persisted overnight and through morning. Lots of tightening throughout the day. Vanicream helps. Peeling begins.
Day 18 (July 11) Day 2 post-application: Woke with terrible headache. Slept better but had to take pain meds to do it. Woke at 3:30 to reapply Vanicream. Went right back to sleep. Very tight. Larger areas are beginning to lift but too stiff to peel and will bleed if pulled. 10 minutes after shower skin pulls incredibly tight. Took about two hours after shower to stop stinging. First application of Vanicream two hours after shower stings. Later in the night applying doesn't hurt. A few ulcers forming inside my cheeks. Had trouble falling asleep. Got up twice to apply Vanicream.
Day 19 (July 12) Day 3 post-application: Very tight again all day. Most uncomfortable day post app so far. Large sections coming loose but still attached in places. Vanicream lotion helps a lot. I wasn't able to apply constantly all day because I was out for a few hours. The longer I was out and went without Vanicream, the more painful and dry I was. This seemed to make bigger chunks form though. Late in the evening I was able to work several large pieces loose and get them off by rolling and twisting. (If they are left too long they do begin to smell bad.) Shower felt good and I could move freely but tightening started about 10 minutes afterward. Some seeping still happening. Bleeding in a few small spots.
Day 20 (July 13) Day 4 post-application: Very hard day. I expected to be almost done peeling. For every spot that has peeled, more seeping has happened. Swelling in my cheeks, around mouth, and across chin. Tight after ice packs are applied and so tight after shower it feels like I'll split open and bleed. Long after shower still tight and throbbing. Stinging. Feels wet and looks dry and is dry to the touch from seeping having crusted over. Was extremely fatigued through the day. Didn't sleep soundly last night. Could feel seeping from chin. Used hydrocortisone instead of Vanicream and it also burned. Finally stopped after about 10 minutes. Seeping was somewhat less overnight. Slept hard in one position with pain med.
Day 21 (July 14) Day 5 post-application: Somewhat easier than yesterday. Washed after getting up and used hydrocortisone. Washed early afternoon and tried Vanicream. Both absorb very quickly. Swelling in the afternoon and evening. So much stinging after shower I took pain pill early. Nose is almost completely peeled and smooth. Parts of cheeks are smooth. Both sides of nose and all of chin keep seeping and recrusting. Very frustrating. Chin is now tightest part making it hard to eat and talk.

After treatment: Days 22-28

Day 22 (July 15) Day 6 post-application: Chin is still scabbed and very tight. The longer the day went on the more swelling I felt. Eating is still hard. Several spots cracked and bled. Stiffened after shower and stung horribly. I used Vanicream on left side of face and chin and hydrocortisone on the right side before bed. Vanicream stung much less. Hard to fall asleep. Woke a few times through the night.
Day 23 (July 16) Day 7 post-application: Chin is crusted. The spot I first noticed before derm visit on left cheek is tender and still scabbed. Small red spot right side of nose on cheek before derm is also scabbed. Nose is fairly clear. Corners are almost clear. This was first morning Vanicream didn't burn with first application. Rolled off a few pieces in shower. Stung after shower. Vanicream after shower stung as well. Noticed today I have a half dozen red spots that have appeared on my chest. They don't itch. I’m just over it.
Day 24 (July 17) Day 8 post-application: Felt much better. Stiff cheeks and a little peeling beside nose and in the two trouble spots. Chin peeling quite a bit. The only pain is on chin. Able to eat larger bites of food.
Day 25 (July 18) Day 9 post-application: Ate normally. Slept well with Tylenol PM last night. No stinging this morning. A little stiff across cheeks and very center of chin under lip hurts if I stretch it. Got a lot rolled off chin in shower. Just a tad stiff after shower. Only spot that hurts at all is dead center of chin directly under my lip. Hard scab there.
Day 26 (July 19) Day 10 post-application: Scab under chin is smaller. Red area has faded and is a little smaller. No sleep meds. Eating normally with no pain.
Day 27 (July 20) Day 11 post-application: Scab almost gone. The only sensation I have now is a little twinge when I stretch my mouth wide enough to yawn. Redness is fading.
Day 28 (July 21) Day 12 post-application: Sleeping normally. A little lingering headache. Redness is still fading. Last of the scab is gone.

After treatment: Days 29-35

Day 29 (July 22) Day 13 post-application: Tried sunscreen on red area. Was terrified it would burn. Haven't had anything but Vanicream on it since post app. Used Blue Lizard spf 30 for sensitive skin. Didn't burn at all. Feel completely normal. Slight headache early in the morning. Redness is fading. Indoor lighting makes it barely noticeable. In natural light, it's still visible.
Day 30 (July 23) Day 14 post-application: First day of vacation. Used Blue Lizard spf 30. No burning/stinging. Treated area grew bright red outdoors in the heat. Stayed under hat and reapplied sunscreen several times through the day.
Day 31 (July 24) Day 15 post-application: Red area is still very defined. Used Blue Lizard spf 30. Umbrella. Bright red in heat. Headache.
Day 32 (July 25) Day 16 post-application: Same as day 31
Day 33 (July 26) Day 17 post-application: Bright red in heat. No stinging, just red and very well-defined.
Day 34 (July 27) Day 18 post-application: More fading. Pretty much the same as day 33. No headache today. Drank lots of water all day.
Day 35 (July 28) Day 19 post-application: Heading home from Florida. Used Blue Lizard spf 30. Less red today.

After treatment: Days 36-43

Day 36 (July 29) Day 20 post-application: Home. Using Vanicream to help the dryness of new skin. Feels a little tight after washing. Still fading.
Day 37 (July 30) Day 21 post-application: About the same as day 36
Day 38 (July 31) Day 22 post-application: No significant change from day 37
Day 39 (August 1) Day 23 post-application: Redness continues to fade
Day 40 (August 2) Day 24 post-application: Similar to Day 39
Day 41 (August 3) Day 25 post-application: Similar to Day 40, more fading. Makeup covers easily. I do see a lightly defined red area without makeup.
Day 42 (August 4) Day 26 post-application: Still notice a defined red area under makeup and in the heat.
Day 43 (August 5) Day 27 post-application: Feel like I’m ready to go back to school Monday without the redness being too noticeable.

In summary

Using Efudex cannot be succinctly described, and no one can predict the results each subsequent treatment will produce. One Efudex user chronicled his experience and documented his Efudex journey through photos. He, unlike many others with whom I have talked, was duly warned by his doctor that the results during treatment could be quite horrendous. I have found that large numbers of Efudex patients are provided with little or no warning regarding the worst-case scenarios during and immediately following treatment. In fact, it seems quite common for dermatologists to compare effects to a “bad sunburn".

My history of sun damage will lead me to remain vigilant with sunscreen use, sun safety, and regular self-checks of my skin. In my preteen years, I set the stage for a long string of events that will hopefully now prevent others from following my lead. Every difficult morning and each sleepless night spent using Efudex is worth the end result. Each time I have treated, my dermatologist has been pleased with the outcome. Areas of concern have been eliminated with new, healthy skin taking their place. Granted, skin cancer is a never-ending journey. New spots will continue to appear, Efudex will likely be prescribed, and I will repeat the process.

hi. great post April, so much valuable information. It is true, when given the script for efudex, there is no true explanation what to expect during the weeks of treatment. This my second time, now at day 16, my face & forehead is harsh, pain, redness, blisters, scab over, some starting to peel. I would like to post a couple pics. thanks again for putting yourself out there. cheers

You're so right about our doctors. They really don't seem to know exactly what to tell us as far as expectations. It's quite a sticky situation when you head into things blind. April, thanks for sharing your efudex experience. I'm currently in day 17 of a 21 day application now and have used your post to track my progress as well as set my expectations on what is to come. Like you, my biggest pain point thus far has been my chin. I have done levulan (blue light therapy) annually over the past 11 years with diminishing effects over that time. After my long-time dermatologist retired a few years ago, my new dr suggested efudex. I googled it and saw photos of others experiences and kept putting the treatment off. Maybe next year...we'll see. After my 6th mohs surgery on my face and head in the last 18 months, I finally gave in to my dermatologist's recommendation. By comparison the efudex is much more intense than the light therapy ever was and from your post, the road to "normal" skin again for me is still quite a ways away. I can't say that I'm looking forward to future efudex treatments but I realize it's probably part of my future treatment plans. I had my first basal cell surgery at 24 and in the 28 years since, I've had more than 50 surgeries. It's probably time to take a different approach.

 @david...I'm blown away by "more than 50 surgeries!" You've definitely been through the worst of the worst. I'm so sorry. Isn't it amazing how such small movements can be so painful around the mouth and chin? It's so difficult to explain in words to anyone who hasn't experienced it. Blue light therapy has been on my doctor's list of treatments for me for a while, but we have yet to try it. Efudex seems to be doing the trick. I'm glad you're giving it a try and hope you get fantastic results. I will say that each year since my second year I have noticed fewer spots on my chest. I have overall sun damage, but my AKs seem to be becoming fewer and farther between. I'm glad you mentioned that Efudex is more intense than blue light. That's helpful to know for members who are debating trying one or the other. Like you, I realize it's a never-ending cycle. Wishing you the best as you head into healing! Noticed your link to this piece when I was posting my own Efudex story. Still not sure how to have the self discipline to keep from picking at the scabs. I think I sort of do it unconsciously by "playing" with a little guy which before I know it is half off, and then I have to finish the process! Never to happy about it afterwards unless they are TOTALLY ripe for and ready to come off. I think maybe I need to use more Vaseline.

@ronnigordon...It sounds a little silly coming from a 44 year-old, but all I can hear is my mom telling me as a kid to stop picking because it will leave a scar. My sister and I were bad about picking at mosquito bites. Those words have stuck with me all these years, and I hear them every time I'm covered with healing scabs from Efudex. I agree with you. You really may need to step up the moisturizing. Vanicream really helps my skin stay soft, and it helps work those loose flecks off much faster than if I let my skin stay dry and irritated. April, thank you so much for sharing. My doctor has prescribed this cream for me to use for 5-6 days, but I have to admit I'm freaking out about it. I teach school, and do NOT want to go to work (or in public) looking like a burn victim. I know it sounds vain, but it's just me being honest. I'm 57, red-headed, fair-skinned, and grew up sunbathing without sunscreen...I think you get the picture. What do you recommend, based on personal experience?  @paige-tucker...I think we are the same person! I'm also a teacher and have fair skin and reddish hair. I'm actually surprised the prescribed time is only 5-6 days. If I had to guess, I'd say your damage must be very mild or the time would be much longer. Most of us are told to use for at least 2-3 weeks. Which area are you treating? I've treated my lips during school and am about to spot treat a spot below my lip before school is out for spring break. I typed up a letter explaining things to my parents so everyone would be prepared for the effects. It didn't make it less uncomfortable, but it certainly helped the kids understand somewhat. If you are still feeling unsure, talk to your doctor and see if waiting until summer is a viable option for you. I know it's a hard decision. Thinking of you! Oh my goodness I had no idea how awful your experience was. I personally feel sometimes doctors "overlook" or glaze over what it's really like. I think they do this either because they deal with it so often it's "normal" to them or they don't want to freak you out. I was having pain mid thigh (below where I had lymph nodes removed) and was freaked out by it. My surgeon told me she put small metal clips on the vessels when they removed the lymph nodes and they could be hitting a nerve or it could be scar tissue. I was a little thrown. I said, "How come you didn't tell me this right away?" She said everyone's experiences are different and it didn't even occur to her to say anything because they do that all the time when they remove lymph nodes. I know this is getting long (I apologize), but given your experience would you use the Efudex again? I was also wondering what brought about your doctor wanting to do this treatment? Did you have a lot of spots to "get rid of"? I am curious why do this cream versus other treatments? It seems so painful. I am so sorry for all you've gone through. Hang in there. You are beautiful and strong.

 @moonmomma....I don't know what some doctors are thinking. It really seems like they would want to keep us updated on all the decisions they make as they make them. I'm so sorry you have had to deal with this and even more sorry you weren't told up front! I have used Efudex yearly since 2014. The first time was an eye-opener for sure. I had no clue what to expect and was floored. I was told it would feel like a bad sunburn--not even close. My doctor suggested Efudex a few times prior to going ahead and prescribing. I battled with precancerous spots for years. Each visit, I was having several spots frozen off. She finally suggested that this cream could take care of a large area at once. I was glad to try it. Even though it's tough, it really does do the trick. I have treated my face and my chest and do spot treatments when necessary. I'll continue to use it when needed. I prefer it to having more and more scars-- especially around my mouth. Thank you for your kind words and I hope you are doing well now! Hi April, Thank you for an informative piece on this treatment. I have recurring basal cel for the past 15 years. Many surgeries and I keloid. I tried Imiquimod and had a reaction (extreme fatigue, mental fogginess) so we stopped that. I was given Efudex. I tried it for less than two weeks on my shoulder. I was again exhausted, ambivalent yet weepy, and again in a mental fog. I just came from the doctor. He insists that Efudex doesn't cause that type of reaction and wants me to try it again. Did the fatigue wear off after a few days for you? Do I just push through it? I really don't want to because I just wasn't myself but if it's short term I suppose it's better than surgery (?). Although with all my scarring what's a few more? If I knew he fatigue/fog was only short term, I'd do it. But I can't imagine living like that for a few weeks.

 @myhoney...I wish I could tell you without a doubt that the fatigue many experience is short-lived, but there is no normal with Efudex. It seems we all experience varying side effects and varying degrees of each. I struggle with fatigue for the better part of treatment. It does seem to be much worse in the first few days. There are other side effects that seem to take priority as the weeks go on. That might be the reason I don't notice the fatigue as much. I am so sorry you are struggling with all of this. I know it's a mental strain and much harder when your doctor doesn't give you a heads up about possible side effects. I can tell you this, though. You are not alone and don't have to go through this treatment by yourself. There are many out there finding themselves asking the same questions as you and wondering if it will ever end. We are here for you any time you want to know what to expect or any time you wonder if what you are feeling is typical for Efudex users. Hi April, Thank you for sharing your story. It helped me through my Efudex experience. I am at the recovery stage now and look very much how you did at day18. After trying a couple creams that stung too much for me to bare, I finally found one which completely soothes the skin and does not sting AT ALL. It is called Avene Skin recovery cream by Eau Thermale. It is a bit expensive $30AU for 50ml but it is so soothing. Hello April I was just diagnosed yesterday start treatment in about 2wks. Thank you for the timeline of your experience. All my Dr. told me was it will get worse before it gets better. My initial treatment is for 3 weeks so now I’m not looking forward to it at all but it will go better with the knowledge I gained from your article. Thank you again.

Good to see youre face healed fine. Mine is seems to be now discolored in different spots permanently 3 months post treatment

I know the first time I treated, I was sure my skin would be red and patchy forever. After several months, you couldn't tell where I had applied the cream. I have occasional flareups of redness, but that's only after I get too hot or when I have a sinus cocktail shot. I'm 2 weeks beyond a 3 week efudex treatment on my chest. Still red. Still red lumps. Is this normal? thanks!

It's not uncommon to have some redness for a few months after the healing stage feels complete. You may have periodic flareups of redness when you exert yourself, get too warm, or take certain meds. (I have some redness in the treated area if I take a steroid or have a sinus cocktail shot.) If the areas that are red and raised didn't go through the erosion phase, you may want to contact your doctor and see if further treatment is necessary. Did the area blister and peel? Thank you so much for this detailed description of your experience. I am on Day 11 of treating my bald head. Your experiences help me know better than anything else I have read that what is going on is normal. The killer is the headache and just overall bad feeling, partly from lack of sleep. Definitely NOT just a severe sunburn type thing. I recently retired and am glad I am not having to work. I think I would be useless. Thanks again.

Hi, just to say thank you for this blog / write up on the effects of Efudix,my doctor never told me about much just to put this on the problem parts of my face that is nose, cheeks mid forehead, first 2 weeks no problem i am on 3 week with one more week to go,i go check up doctors on 4th oct 21,my doctors are not pro active these covid days, you have to follow it up your self!! So i have started some headaches now bleeds and scabs most on nose and pain, but i do have high pain tolerance thank the lord, maybe with my martial arts and how i think! I don't like to look in the mirror as i always had clear skin all my life, so this was shock i was not sunbather and always hat on sun cream when out in summer so who knows ,you see people striped off sunbathing i think how! Anyways i am so glad of your write up your a brave woman to do this, and i am glad of it, as took some pointers from your info you put on this site i ordered the vanicream post treatment and i do keep out sun,i am 59 white male pale skin blue eyes, so maybe that's something to do with it who knows, once again thanks for this i appreciate it ,i know what to expect and also ask my doctor that's the main one so i got the information to ask <a href='http://now.Still' target='_blank' rel='noopener noreferrer ugc'>now.Still</a> a shock but i not had the pains and the rest like you seem to have! once again bravo for this information. <inline-mention username="April Pulliam" user-id="2086092"/> your the one

Thanks for that April ,like you i took photos start to end i hope, on the last week i did not sleep well last night first time with headache cant lie on side of face as sore and hot flushes ,here in uk weather cool damp so when i stick my face outside its nice. So it is bit sore looks red raw some peeling i try to pick them!! i know! i got he cream you mention on here Vanicream, so do i start that after i come off treatment ? can i use any cream now ,if i go out i put on zinc cream sudacream only to cover the red when i go to work or shops i try to stop in and work from home best i can. I go to see the dermatologist in Nov sometime. Went to the doctors for check up she was shocked to see my face like this ? i told her it can go this way if more damage you can not see under the skin ! she wanted to put me on anti biotics and stop treatment and use some Hydrocortisone cream as well ,i said no i need to go this 4 th week and then see what happens not sure why anti biotics ? i drink lots water try keep off face picking ,it is sore now .I can stand this but i think i don't want to go though this again, it does take physical and mental courage. Yes like you say the doctors just don't seem to have a clue on this and also telling you what happens you go in blind i did and i am so glad i found your blog and photos they helped me a lot to go on with it. So does it hurt more when you stop the cream? this Sunday 10 Oct is my last day 1 morn 1 night, yes i do it 3 hrs before i go bed. Then i hope its been worth it and winter here so my face can get used to it then spring summer and yes sp50 sun cream face all times winter and summer! i do like outside life always have, but never sunbathed i run a lot walks in hills but always put cap and cream on ! and once again thank you for this if i can i can put my photos up if anyone wants to see my progress if it helps, thanks again April.

I try lying on my back as much as I can to keep my skin from sticking to the pillow when I am using the cream on my face. I am sure the cool weather does feel nice! I am glad you chose this time of year to treat. Most doctors encourage patients to treat in the fall and winter. Summer heat can be a real challenge. I strongly encourage you not to pick at your skin. I know how hard it is. It's very tempting to peel it, but try hard not to touch it. Your skin is becoming raw and you don't want to transfer germs to raw skin. You are very close to the end. If your skin is seeping clear fluid or beginning to bleed in some spots, you are likely finished. Many patients this stage, the erosion phase, before the prescribed time is up. Once you finish completely, you will want to start using Vanicream to help soothe the skin and keep it moist. I wouldn't use any products on the skin until you have applied your last dose of the cream. The healing stage is definitely more painful for me than actual treatment. Your skin has come to depend on the cream and it goes through a stage of withdrawal. It becomes kind of angry. Your skin may become very dry. When it does, you may find it difficult to move your face. That's where using the Vanicream comes in handy. I tend to apply regularly throughout the day to keep it from becoming too dry. Once you start the healing phase this weekend, to keep from being tempted to pick at your skin, I suggest you try to moisten your skin in the shower and while it's wet, roll the dead skin off very gently. Don't tug and don't force it. This also seems to make the healing phase move along more smoothly. I would be very careful applying sunscreen. It may sting right now. Once your skin begins to heal, sunscreen may not feel so uncomfortable. i found this one not sure if anyone know but its info on what we are doing so might help ? A four-day topical combination regimen of 5-fluorouracil (5-FU) and calcipotriene removed almost 90% of facial actinic keratoses – significantly more than with 5-FU monotherapy, in a randomized, double-blind controlled study. Calcipotriene (Dovonex) is a synthetic vitamin D3 derivative approved by the Food and Drug Association for treatment of scalp psoriasis. But calcipotriene is also an immunomodulator that induces thymic stromal lymphopoietin (TSLP), which suppresses the growth of early stage skin cancers, said Dr. Shawn Demehri, of Harvard Medical School, Boston. Dr. Shawn (Shadmehr) Demehri Dr. Shawn (Shadmehr) Demehri To determine whether short-term TSLP induction could reduce AKs, he and his coinvestigators randomly assigned 131 men and women who were at least 50 years old and who had at least four AKs on the face, scalp, and/or upper arms to apply 5% 5-FU cream mixed with either 0.005% calcipotriene or Vaseline to affected areas twice daily for four days. The researchers counted and photographed the AKs at baseline and at subsequent follow-up visits. The average age of the patients was 70 years, and 82% were men, said Dr. Demehri, who reported the results at the annual meeting of the Society for Investigative Dermatology. The combination of 5-FU and calcipotriene was associated with an 86% reduction in the number of facial AKs, compared with a 26% reduction among patients who used 5-FU monotherapy (P less than .0001). The investigators observed equally dramatic differences in efficacy at other body sites. On the scalp, combination therapy reduced the number of AKs by 76%, while 5-FU alone reduced the number by only 6%. On the right upper arm, the dual regimen removed 70% of AKs compared with 10% for monotherapy, and on the left upper arm, combination treatment removed 80% of AKs, while 5-FU alone removed only 16% (all P values for these differences were less than .0001). Notably, patients did not experience pain or crusting after using the combination cream, said Dr. Demehri, who is also a principal investigator in the department of dermatology and MGH Cancer Center, Massachusetts General Hospital, Boston. Nicotinamide is a water-soluble form of vitamin B3 or niacin. It is made in the body by eating niacin-rich foods such as fish, poultry, nuts, legumes, eggs, and cereal grains. Nicotinamide supplements are used to treat skin conditions and niacin deficiencies. Recent studies suggest nicotinamide may protect against some forms of skin lesions in patients with sun-damaged skin. Additional studies are needed to confirm safety and effectiveness across different types of skin cancer and in different people. In addition, the protective effects of nicotinamide against UV exposure does not mean that it protects against sunburn. To prevent skin cancer A large study found that taking nicotinamide can reduce the risk of getting certain types of skin cancers. A few small studies suggest it may also reduce the occurrence of rough scaly patches. Additional long-term studies are needed. To treat acne and other skin conditions Nicotinamide is used as a medicine for treating skin conditions such as acne and rosacea. he prevention of common skin cancers and precancers is possible by taking an inexpensive, widely available, oral pill twice daily. The pill—the vitamin B3 supplement called nicotinamide—cut the rate of new squamous-cell and basal-cell skin cancers by 23% compared with placebo after 1 year among patients at high risk for skin cancer. Nicotinamide also reduced the risk for developing actinic keratosis, a common precancer of the skin. The results of the phase 3 ONTRAC skin cancer prevention study were presented at ASCO 2015. These findings have the potential to lower healthcare costs. In the United States, skin cancer accounts for approximately $4.8 million annually. The investigators emphasized that these results were achieved in individuals who previously had skin cancer and were thus at high risk for new skin cancers. In addition, the investigators emphasized that nicotinamide is the form of vitamin B3 that should be taken for prevention—not other forms of vitamin B, such as niacin—and that continuous treatment is advised. Worth a look up on this and that other treatment !

update i am off treatment did not get any pain but i was not as bad as you April so sorry you got it bad,did near 4 weeks face and some on wrist 3 little red marks ,nose is still 3 scabs on it yet rest skin seems to be healing some red,does seem like its going. I am on Vitamin E and i also put one one skin pop capsule and spread on the skin 2 x a day unless i go out.I am also taking Nicotinamide 1000mg a day and also use skin protection when out was thinking to use a mineral sun cream next summer ,i always did wear caps sunglass that are UVA and B filter,as i said was not a sunbather found a few people like me that say the same maybe more UV output on sun as its getting older. i will let you know what happens i am due to see skin specialist sometime in November,so hope its over !!! dont want this again noooooo.

I recently commented under a different post, but my recent experience lends to this post, and i wish i had of read this site before undergoing efudix, because my experience with applying efudix to my entire face has been horendous. After 3 doctors seemed to recommend efudix with such ease and no warnings, i eventually gave in and began to apply it to my entire face, as the doctors had prescribed. I was hesitate, but eventually took the leap. Big mistake. By the 13th day of application, my skin felt burnt, sore, was red raw with hardened dark patches blotched all over my face, with a think band of hard patch across my nose and my chin. I thought 'oh my god, what have i done'. While one doctor recommended the application of efudix for 14 days, the other said 2-4 weeks - i stopped on day 13, enough was enough. On day two after stopping efudix i woke with a swelling under my eyes. I phoned one of my doctors, who said to apply moisturiser - nobody had told me that that was needed after ending the application! I went back to my other doctor (i have a local GP and skin cancer doctor, both deal with my skin cancer checks and care - the GP is easier to get into than the skin cancer doctor.) I was told i had had a bad reaction to the efudix, even though i looked as horrid as all the other images of efudix use i had google searched. The doctor prescribed cortisone cream and prednizone. It is now day 19 (day 6 of recovery after 13 days of applying efudix), and i have cried every day, sometimes all day, moiturised everyday, morning, noon, night and sometimes inbetween. The dry patches have sloughed off in the moisturiser. If anything if have learnt is - moisturise moisturise moisturise - that way the scabbing slides off instead of dry peeling which can bleed and hurt more. If you are on the fence about using efudix on your face, and you are reading this to make your decision, please take my advice and do not use efudix on your face. If you have a diagnosed bcc, it can be spot treated using efudix, or cut out. The complexion of my face was good before using efudix, although i realise i am early in the recovery phase, but i look bright pink and blotchy, and cannot imagine ever looking my normal colouring again. But thanks for this site, it has been consoling to hear that i am not alone in traumatic use of efudix, which i will never ever ever use again, and now believe that i did not really need to use all over my face in the first place. But when three out of three doctors seem to have no trouble suggesting it with such ease, what is a girl to do .. NOTE TO SELF: TRUST YOURSELF MORE. Just because you are not a doctor does not mean you do not know what is best for yourself in the long run. I should have listened to me ...

Spot treating your face is infinitely easier than treating a large area. The redness you are experiencing will subside. I know it feels like things will never return to normal, but they will. Don't be surprised if you have some episodes of redness recur over the next few months when you exercise or get hot. It's not uncommon for me to have redness flare up when I take an oral steroid or have a sinus cocktail shot. I am sending you lots of hugs! I know what you are going through! Thanks so much for sharing your story, I've been doing a lot of research and found your day to day notes very helpful. I've just started my first efudix field treatment last night (I've just completed some spot treatments on 5 BCCs on my back which are now in the blister phase). I've had many BCCs burnt and cut out over the years, and during my last MOHs surgery, the doctor suggested field treating my whole face, chest, arms and back. I've grown up and still live in the Caribbean so it's tricky to figure out a time to do the treatment (wish it were winter and I could just cover up!) and my work involves being face to face with people daily, so I've tried to schedule the treatment during some downtime.  I've started with my full face and full arms as I can't see another big chunk of time that I can stay home any time soon- perhaps this is ambitious?  I'm meant to apply efudix to my face once a day and arms twice a day for two weeks. I have a few questions that I was hoping someone could provide clarity on, as like many of you, I didn’t quite get the tangible answers from my doctor despite my specific questions.  First, how much of the cream should I be applying. I’ve seen online that for the full face, it should be about 4 pea sized amounts, but I can’t find any information about the arms and hands.  Also, should I be rubbing in the cream or leaving more of a thin white layer? For the isolated spots on my back I did a thin visible layer, but this seems like a lot for field treatment?  In terms of moisturizer during treatment, should I try to use moisturizer on my face during the day as the efudix is only going on at night or is it better to not put anything on during treatment and wait until the healing phase to begin to moisturize. Same question for sunblock.  Finally, does anyone have an experience with efudix and exercise? I find exercise helpful for my mental health, and knowing i’ll essentially be stuck at home over the next month, i’m hoping to continue being able to exercise. Is this more of a personal preference in terms of how painful it can get with sweating and increased body temperature, or is the recommendation that you should stop exercise?   Thanks so much in advance for any guidance. I am so sorry you are dealing with this treatment, but you are definitely going about it the right way by asking for advice up front. As far as application goes, a very thin layer will do it. I typically rub it in just enough so that I don't see a white film anymore. The cream tends to absorb quickly into the skin. A little bit really does go a long way. When considering products like moisturizers and sunscreen, you may run into varying opinions and advice from other users. From what I have always been told, it is not advised to use any products on the treated area during the application phase. My doctor has always told me to only use water--not even soap-- to cleanse the area as long as I am still applying the cream. It's not easy to adhere to those instructions, but I try very hard because making sure the cream is effective is a priority for me. I think you will find lots of advice on exercise. Some people do have side effects that leave them feeling nauseated and lethargic. If exercise is something you feel like doing, I would definitely suggest it. Prepare yourself for some stinging and burning in the treated area if you sweat though. Heat always tends to aggravate the tender skin. I am including links to a few articles you may find useful. I am using Efudex for the first time on a patch on my left forearm for Bowenoid Actinic Neratosis. I just saw one person post journey and it looked extremely red. I don’t know what she had but I am curious whether that is what I’ll have to contend with. My doctor did freeze out my patch before the first use of Fudex. Anyone else?

First, let me thank you April for this very informative article. My dermatologist today prescribed Efudex 5% which I am to start in 2 weeks; he made very light of the side effects. After reading this I am not sure that I want to put myself through this as I have Connective Tissue Disease. I am going to get a second opinion with another dermatologist even though I like mine.

Thank for this information . . . this has helped a lot with understanding this treatment [on the side of my face - temple area] as my doctor did not explain anything whatsoever about the treatment except the actual '3 week course of treatment' and that it will sting a bit . . . . . but due do this lack of actual treatment info which is vital to know, from my doctor as to what to expect I knew nothing what-so-ever with the result is that well into the treatment and all the symptoms of the treatment that is quite horrendous and looks horrendous and the pain, the discomfort and and etc etc left me in a panic and even hopeless in despair . . . . . but now I have done some research and listened to the relevant info on the preceding videos I watched etc and coming to this informative website as well has been great for helping me now to understand this treatment and I can "relax" now and let the treatment take its' course with all it discomforts that it is meant to take place . . . so thank you!

 This is the most pathetic post I have ever read or seen! Your dermatologist told you to use only in small patches at a time. Size of your palm. If he or she didn't give you the easy sheet how to use they should have their practice closed. I have been using this med for 20 years with great results. Very very poor decision on your behalf to do your whole face! WTF!!

 Julie I am 60 years old, blond hair, blue eyed, and very fair skin. I have been dealing with basal cell and keratoses for the last 35 years. Most of the damages took place when I was a young boy. Parents back then didn't have options or knowledge of sunscreens like today. I am getting ready to start another cycle treatment of Efudex 5% right after Christmas. I was always told by my dermatologist, of 25 years, to only treat small areas at a time, no larger than a silver dollar. Then move on to another hot spot 6 weeks later. Trying to "get it all over at once" you run into horrific problems, like April did. I only need to use Efudex this way every 4-5 years. This medicine is like round up on weeds. You will more than likely have to go through another treatment in the future. Using it in small areas at a time will not give you the feeling that "I will never use that again!" Wayne

Thank you for this website. I never thought I would find community in topical skin cancer chemotherapy! I have used Fluoruoracil off and on for over 10 years. The first two years I would treat my entire face in January. Explosion of sores, but the result was worth it. Now I only need to spot treat areas as they occur. Two weeks ago I found a spot on my chest and was prescribed Fluorouracil. I treated just the one area, but the skin around it indicated there were other precancerous cells. So I am treating my whole chest. It is prickly heat painful, difficult to ignore, with a slight headache controlled with ibuprofen and acetaminophen. Although my husband has encouraged me to stop the treatment I will continue. Too far into it to stop now. Just two more weeks to go. A big thank you to the creators of this site, and to all who share. It really does help.

I went through this 21 day treatment not long ago. It was a little rough, especially toward the end of the 2nd week and in to the third, but it is totally doable and worth it. When I look back on it, I am so glad that I did it. My dermatologist prescribed the cream to me and showed me a picture of the pink color my face should be after the 21 days, without describing what I would be going through. I found some good, positive videos on Youtube for that, though, from others going through it. My advice to others would be to just do it, and get it done. Start marking the days off on a calender, and do not stop short of the period prescribed by the doctor. Your skin will crust over, bleed in spots and come off, but you just keep going. I would also advise that you could still work during that first week if you wanted to, and maybe just take the two weeks off. I went out in public the whole three weeks, and sometimes I would explain to people what I was going through. The toughest part for me, I think, was when I would get out of the shower and pat my face dry with the towel during the 2nd and 3rd weeks. It was painful as my skin dried, and I guess constricted, and even though it was painful putting it on at this stage, it was kind of a relief to get the cream back on. To wash my face, I found it easier to just get in the shower, and kind of lightly splash water on my face. Another thing, when I started out, I used too much of the cream. It doesn't take much, and I didn't run out, but I was kind of worried about it toward the end. Another thing, if you use a CPAP at night, you might want to ask your doctor about it. There isn't any information about that. I just didn't use mine for two of the three weeks. One more thing, my dermatologist really liked that I took pictures of my face through the entire process, so that he could see it for himself. But, if your doctor wants you to do it, get it done now, and don't put it off. It is a little rough, but not too terrible, and you will be glad that you did it. My dermatologist told me I did good.

I posted something earlier, but forgot one point I wanted to make. Efudex, or Flouroricil, is very poisonous to pets. You do not want your dog getting ahold of the tube or licking an affected area.

I’m on day 11 of 28 of tolak once a day… a whole lot more problem areas showing up than I realized I had. Some are starting to scab already. It’s going to be a long few weeks. Hope I can last 28 days

tomorrow is my last of the 28 day treatment. I have my creams, lotions, ice and prescription hydrocortisone… This may well have them worse than my total knee replacement.

My doctor prescribed Triamcinolone Acetonide Cream 0.1% to apply when the itching and burning gets bad. It does help, but I'm going to try the Vanicream as you suggested also.
